We're committed to giving our users more freedom when it comes to choosing the support they need. But we recognise that some will still need a little assistance to make those choices. That's where you come in. You'll go out to people's homes, have the ability to work across a designated community area with suitable access to transport and help them self-assess their needs. Depending on their complexity, you’ll either offer your own advice on the most appropriate services available, or liaise with one of our practitioners. By tailoring people's care in this way, we can be more responsive to their needs and deliver a level of support that will transform people's lives.

It’s important that you're personable and can relate to people from all walks of life – so strong communication and relationship-building skills will be key. Because of the nature of your work, we'll be looking for an interest in social care and what it can achieve. Plus you'll need a good standard of education, including GSCE English or Maths, and a working knowledge of Microsoft Outlook and Word.
