In Essex Adult Social Care, we work together to empower our service users. We do this through Self-Directed Support; working in partnership to give people more control over the support they receive. This innovative approach enables service users to maintain their independence and gives them more opportunities to interact with their local community.
To deliver this new approach to our users we need Team Managers, SDS Senior Practitioners, SDS Practitioners, SDS Facilitators, and Business Administrators.

This team work with people with complex needs who require support on a monthly basis. Partnership working will be key and because of the range of support needed, it’s likely to suit those who enjoy working in an environment that is reactive to prevention and crisis intervention.
Most users in this area have multifaceted needs and require support from both health and social care services. This team will build on the support already provided by Essex County Council and the Mental Health Partnership Foundation Trusts, and deliver a more integrated service to users, their careers and families. This multi disciplinary approach will suit those who enjoy working in a holistic environment.
Partnership working will be key for this team. They help people with complex needs that require support and intervention on a monthly basis. Users include adults with a physical impairment and those with a learning disability, who’ll receive assessments, support planning and regular reviews. This team also supports young disabled adults in transition from Children’s Services to Adult Social Care.
These teams work in close partnership with a range of NHS Trust staff to prevent unnecessary hospital admissions and safely discharge those users who did require hospital treatment. There is a high turnover of work in this area and it would suit people who prefer a fast-paced, task-centred environment.
Here, you will receive referrals from hospitals or Social Care Direct. Like in many of our teams, partnership working will be key (especially with the Reablement Service) to ensure users enjoy a more personalised level of care. It’s likely that this team will have a high turnover of people to support, so it will suit those who are highly organised and enjoy working in a task-centred environment.
This countywide team get to work once an individual’s support-planning objectives have been agreed. The Complex Support Planning Team enables individuals to achieve a unique person centred support plan which leads to greater choice and control.
Using a risk-based approach, which allows for telephone and self-reviews as well as face-to-face interviews, this team will focus on older users as well those with physical and sensory impairments. They’ll respond to both planned and unplanned reviews.
This specialist team is divided into provision and assessment services. Provider services will support assessment of equipment and mobility rehabilitation. Specialist assessment of sensory needs will be provided to any of the other teams within the service through a county wide team.
